Service coordinator job description
The service coordinator assists the prospective participant to become a waiver participant and coordinates and monitors the provision of all services in their service planServices may include medicaid state plan services, non-medicaid federal, state and locally funded services, as well as educational, vocational, social, community and medical servicesThe goal is to increase the participant's independence, productivity and integration into the community while maintaining the health and welfare of the individual.
Service coordinator responsibilities
Act as an advocate for the participant while ensuring their right to choice as outline in their waiver participant rights and responsibility agreement, while respecting their dignity to risk and the right to fail.
Assists the participant in the development, implementation and monitoring of all services in a client-centered and individualized service planInitiate and oversee the assessment and reassessment of the participant's level of care and on-going review of the service planFormally reviewing, and submitting initial and ongoing service plans to the service coordinator supervisor for review prior to submitting to the prospective regional resource center (refer to the service plan section of this manual for specific information on service plan responsibilities).
Provide intensive case management services including but not limited to: crisis intervention, risk reduction and emergency referrals to ensure the participant's health and welfare.
Conduct a minimum of one monthly face to face visit and at least one quarterly visit to the participant's residenceSchedule and facilitate team meetings bi-annually or as needed for participant'sAttend appointments and meetings as necessary.
Link the participants with an appropriate service provider through community referrals.
Assist the participant in securing a community residency that meets their individual welfare and safety needs.
Meet all documentation timelines while maintaining quality of services including but not limited to: complete service plan packages; serious reportable incidents; monthly face-to-face documentation and progress notes; housing requests; transportation records; and team meeting notes.
Participate in trainings at the discretion of the supervisor and strive for professional developmentIncrease their knowledge base related to participant populationReview program updates as administered and implement all changes, regulatory and otherwise.
Required knowledge and skills & materials
Possess a basic knowledge and understanding of public welfare, social security benefits and entitlementsGeneral knowledge and understanding of developmental, physical and cognitive disabilities, traumatic brain injury, chronic medical conditions and/or mental illnessHave knowledge and understanding of the challenges facing individuals who have developmental, medical and cognitive disabilities.
Must have drivers license with reliable transportation (for counties outside of nyc)Must have basic computer skills with accessibility to a computer, printer, scanner and internet service with the ability to email (established home office set-up)Must have strong advocacy skills, ability to multi-task and handle large amounts of paperworkMust also effectively communicate both verbally and in writingMust have the ability to make decisions and problem solveMust possess excellent time management skills.

Education and experience
1Licensed master social worker (lmsw- licensed by the nys education department);
2Licensed clinical social worker (lcsw- licensed by the nys education department);
3Individual with a doctorate or master of social work degree;
4Individual with a doctorate or master of psychology degree;
5Licensed physical therapist (licensed by the nys education department);
6Registered professional nurse (licensed by the nys education department);
7Certified teacher of students with disabilities (certified by the nys education department);
8Certified rehabilitation counselor (certified as a certified rehabilitation counselor (crc) by the
Commission on rehabilitation counselor certification);
9Licensed speech pathologist (licensed by the nys education department);
10Licensed occupational therapist (licensed by the nys education department);
Must have, at a minimum, one (1) year of experience providing case management/service coordination,
Information and referral services and service brokering of community based services for individuals
With disabilities and/or traumatic brain injury;
Individual with a bachelor’s degree and three (3) years of experience providing case
Management/service coordination, information and referral services and service brokering of
Community based services for individuals with disabilities and/or traumatic brain injury.
Any provider currently providing service coordination approved pursuant to the qualifications in the
Waiver prior to the most recent approval is approved to provide services under the new applicationNew
Employees/providers are required to meet the new qualificationsThe grandfathering of current
Providers sunsets when the staff person terminates employment with the current employer.
