The clinician coach will focus on enhancing the clinical management of cmg providers focusing on his or her patient panel in order to achieve optimal clinical and performance outcomesThe coach will focus on optimal accurate documentation to help ensure patients are connected to the necessary clinical resources and followed up on appropriatelyThe coach will also focus on providers with a high mlr, challenges with high risk patients, or those with an opportunity to optimize referral patterns through primary care managementAdditionally the coach will offer education on clinical guidelines and quality opportunities for cmg’s broader provider community and will support the implementation of new clinical workflows in collaboration with the operational practice facilitators.
Responsibilities and essential functions
Collaborate with medical and population health leadership to identify candidates for coaching and expected outcomes
Function as a specialist in coaching to focus on clinical areas of opportunity including topics such as disease management, utilization management, and care plan development
Complete patient chart reviews in order to identify areas of need
Engage with providers to identify opportunities for improvement to promote shared ownership of coaching
Develop an action plan, in collaboration with the provider and team members (as appropriate) including outcomes expectations
Execute on the action plan with an eye towards measureable outcomes
Collaborate with clinical committees to coordinate activities pertaining to clinical care pathways, required education, and continuous quality monitoring and improvement
Develop educational opportunities for all cmg providers on clinical updates
Collaborate with the clinician coach team in the development of project workflows
Independently manage workload assignments specific to designated regions/clinics

The scope of the coaches’ practice will be determined as the role is developed, but the successful candidate will have the ability to manage multiple assignments and new project creation

Experience working in a medical setting
Competency in electronic medical records
College degree required, and licensure required– rn or lpn along with 7 – 10 years of clinical experience
Motivational interviewing experience or certification preferred
Success in coaching or leading adult learners
Working knowledge of excel and powerpoint
