Social workers support the integrated care team by monitoring and addressing social/psychological determinants of health (sdoh) that requires continued communication with varied disciplines both within and outside the organizationThis includes oversight of sdoh screening and follow-up, ensuring that high risk behavioral health patients have access to ready care, expediting integrated care between behavioral health and other service lines, and coordinating connection to community treatment resourcesSocial workers ensure that the behavioral health patient’s care delivery team is well-informed by bridging communication to and from patients, pcps, bhcs, and community behavioral health providers (cbhp) when necessary.
Responds to identified patient sdoh through further assessment and connection to community resourcesMonitors gaps in referral connection and modifies referral accordingly
Helps support food bank deliveries, transportation to services, insurance sign-ups, and patient engagement, as needed.
Supports the psychological well-being of high risk behavioral health patients on an assigned patient caseloadClosely coordinates care with the patient’s medical provider, bhc, and other care team members for monitoring, tracking, and treatment planning
Manage high risk referral process and monitor follow-up needs, including discharge, ensuring roi is completed for all patients in caseload for coordinating care with community providers and hospitals
Provide back up for bhcs for suicide risk assessments and disposition.
Receive, coordinate, and track referrals for mental health assessment and treatment with community mental health providers
Update patient registry within ehr for patient monitoring and reporting to care teams, clinical team leads, and social work director
Function as liaison with mental health and social service partners in the community to coordinate services for patients.
Monitor bhc team schedules and assist with balancing schedules on a weekly basis to better match patient needs with clinical scope (i.ePsychologist vsLpc/lcsw) as well as optimize productivity
Work with dental and medical providers to schedule patients with behavioral health providers in a timely manner.
Works closely with care coordinators though receipt of high risk referrals and update of regular care plan.
Education: bachelors degree in social work (work experience in relevant area may be considered in of degree for students in a social work educational program)
Experience: 1-to-2 years working with patients in a healthcare setting; working knowledge of basic medical terms
Level3=requires intermittent high concentration and high attention; generally, task approach is consistent, but number of steps/variables create causes-for-error without close attention
Sitting 70%, standing 15%, and walking 15% of time
Lifting, pushing, or pulling 5% of time; max 30lbs of computer, paper, and supplies
Machines/equipment (hand/eye coordination):
On-the-job travel: up to 30% of time traveling, excluding regular commute to primary location(s)
