Overview: axispoint health is seeking a behavioral health specialist/licensed clinical social worker that is bilingual spanish!
To provide behavioral health support in identified cases of members who meet enrollment criteria, focusing on members who are high and/or moderate risk with dominating mental health conditionsThis position requires being bilingual in spanish/english.
To provide behavioral health resource support to the care team for members who meet enrollment criteria, focusing on members who are identified as high risk with behavioral co-morbiditiesThis includes a variety of support to members with serious or severe physical illnesses, developmental disabilities, experiencing end of life issues, caregiver burnout or complex psych/social barriers that interfere with obtaining maximum benefits from qualifying programs.

Responsibilities & qualifications:
Responsibilities (key deliverables):
Reviews identified and/or referred high cost/high risk (hchr) cases to formulate an individualized plan of care.
Builds relationship with members through individual or group conferences to assist members and families in understanding and implementing care plans to improve or restore members to maximum medical, social and mental health.
Provides consultation to the care team for members who are identified as high risk with behavioral co-morbidities.
Provide ongoing coordination of community and governmental agency access to services to mitigate members’ barriers to optimal health and well-being.
Assists in improving self-care and self-advocacy
Works collaboratively to develop and carry out a comprehensive care plan to provide maximum benefit for each member.
Identifies, develops and maintains relationship with community, health and welfare agencies and state groups that are involved in the care of vulnerable populations and who may be resources to the program population.
Regular, reliable attendance and timeliness
Other duties as assigned
Licensed clinical social worker from an accredited school or lmhc, rn with specific training in behavioral counseling (no rx privilege needed)
3+ years’ experience in clinical social work or behavioral health field with an emphasis on working with members with behavioral health issues.
Current unrestricted license in the state that they resideRns may be required to obtain additional state licenses
Must be bilingual in spanish
Windows environment (preferably word and excel)
Excellent written and verbal communication skills.
Excellent organizational skills.
If field based, a current ppd is required.
If field based, aurrent valid driver’s license and required level of automobile liability insurance coverage.
Thorough knowledge of behavioral health issues and the application to case management including complex case management.
Understanding of family and group dynamics.
Familiarity with change behavior techniques.
Knowledge of government and private organizations, resources in the community, laws, regulations and policies that govern clinical social work.
Skill in establishing rapport with members, families, and local resources (medical, social, civic, legal, religious organizations)
Ability to communicate and execute work effectively.
Preferably a member of the local community.
Functional/technical knowledge, skills and abilities required:
Experience in a windows environment
Basic typing and computer skills along with knowledge of word and excel.
Bilingual in spanish/english required
This position is eligible for the follow job grades advancements:
M0- new hire in training (0-12 months tenure)
M1- tenure > 12 months and meeting performance and quality metrics
M2- tenure > 12 months and meets and exceeds performance and quality metrics (above average performer, mentor.
M3- meets requirements for m2 and acts as a preceptor and/or team lead
