Pmp seeks an experienced public health and/or health care operations professional with evaluation and data analysis skills to serve on our population health teamThis is a full-time position, working remotelyThe population health manager will work closely with the director of population health in driving and managing pm pediatric care’s growing population health and clinical integration strategy across service lines and transformation to value-based care delivery and financial modelsSuccessful candidates must excel at managing processes across the interdependent clinical and operational teams of a large health care organization, including structuring collaborative multidisciplinary team meetings; have deep knowledge of performance expectations under payer contracts; and be experienced with change management strategiesKnowledge and passion for improving pediatric population health is a must description responsibilities:
Developing internal and external materials that promote the strategic direction of the organization towards a population health model.
Researching best in class population health tools, technologies, and organizational structures, and integrating them into the organization.
Documenting key processes, work plans, and timelines associated with the development of population health interventions.
Coordinating with hit team and analysts to map data informatics, set expectations for performance monitoring and ensure timely availability of metrics.
Serving as a key contact in organizing and managing energetic but focused multidisciplinary team meetings across departments and with regional clinical leaders to develop work plans and performance metrics for population health activities.
Engaging internal and external partners to monitor population health pilot activities, identify problems with work processes, and ensure strategic needs and partnership goals are met.
Complying with all payer and regulatory requirements associated with population health.
Master`s degree required, preferably mph, mha, or mba
Minimum 5 years of experience in managing operational programs, process improvement and customer relations within the context of a health system, medical provider group, health insurance organization or related healthcare organizations is requiredSuch experience will reflect a successful track record in the following areas: 1) implementation and management of complex operational systems, 2) healthcare provider payment mechanisms, 3) strategic planning, 4) team building, 7) customer service 8) performance improvement and 9) data analytics.
Experience working with subject matter experts, individually and through committees, to assess, define and implement focused change management also required.
Preferable experience working in a pediatric health system.
